9月30日、厚生労働省は、中央社会保険医療協議会(中医協)において費用対効果評価制度における薬価引下げの拡大を提案し、中医協はこの方向性を支持しました。これを受け、JPMA、PhRMA、EFPIAは、私たちの懸念を表明し、日本政府に再考を強く求めるため、本共同声明を発表します。

7月、私たちは、高市政権の「経済財政運営と改革の基本方針2026(骨太方針)」および「日本成長戦略」を歓迎しました。両方針は、革新的医薬品産業を戦略分野と位置づけています。そのうえで、特許医薬品市場を主要市場並みの年率9.6%成長に回復させる目標を掲げ、初収載時と特許期間中のイノベーション評価を進めるとしています。

この目標の実現に貢献するため、私たちは8月26日の中医協での意見陳述において、具体的な施策を提案しました。一つは、購買力平価で調整した米国価格と整合する水準を目指した、初収載時の薬価ルールの見直しです。もう一つは、市場実勢価改定、市場拡大再算定、費用対効果評価制度など、特許期間中の医薬品の薬価維持を妨げる制度の凍結です。費用対効果評価制度は、実際には主に薬価を引き下げる手段として運用されてきました。評価を終えた特許医薬品57品目のうち45品目(79%)で薬価が引き下げられた一方、引き上げられた品目はありません。

並行して、私たちは5月から8月にかけて行われた費用対効果評価制度の「客観的検証」に誠実に参画し、制度面・技術面の未解決の課題を指摘してきました。にもかかわらず、9月9日に中医協に報告された検証のとりまとめは、評価に「一定の妥当性はある」と結論づけました。私たちはこの結論に同意できません。結論に至る検証の客観性には疑問が残ります。

9月16日の中医協での意見陳述では、この結論の根拠と適用範囲が明確でないことを強調しました。そのうえで、制度面の議論が尽くされていないことから、費用対効果評価制度による薬価引下げを当面凍結するよう求めました。しかし、検証の結論を確定したものとして扱われ、価格調整(引下げ)範囲の拡大を前提とした議論を進めることが提案されました。これほど重要な決定には、より丁寧で透明性のある議論が必要です。

こうした経緯を踏まえ、今回の提案と中医協による支持を、私たちは深く憂慮しています。費用対効果評価制度の拡大は、特許期間中の薬価をさらに引き下げるものであり、特許医薬品市場の成長という政府の目標とは両立が難しいと考えます。また、骨太方針および成長戦略が示した前向きな方向性との整合性について、実質的な議論が一切ないまま本提案が進められていることを懸念しています。

高市政権の目標を実現するには、イノベーションへのインセンティブを強化し、日本が革新的医薬品に対して応分の負担を果たすことが必要です。米国の最恵国待遇(MFN)価格政策が、日本での新薬の開発・上市のインセンティブを大きく変えつつある今、その重要性は一層高まっています。特許期間中の薬価引下げをさらに進めれば、ドラッグ・ロスの深刻化や、投資先としての日本の魅力の低下につながるおそれがあります。これは、革新的医薬品産業を経済成長の戦略分野として育成するという政府の方向性に相容れないものです。私たちは厚生労働省に対し、本拡大を実施せず、政府の政策の方向性に沿って引き続き十分な議論を行うよう強く求めます。

Joint Statement on Proposed Expansion of the
Cost-Effectiveness Assessment System

The Japan Pharmaceutical Manufacturers Association (JPMA), Pharmaceutical Research and Manufacturers of America (PhRMA) and European Federation of Pharmaceutical Industries and Associations (EFPIA) collectively represent the world’s leading biopharmaceutical research companies operating in Japan. We support an innovation ecosystem that encourages the discovery of medicines that transform lives, and where we work as trusted partners with the Japanese government so that patients can access the treatments and cures they need.

On September 30, the Ministry of Health, Labour and Welfare (MHLW) proposed at the Central Social Insurance Medical Council (Chuikyo) to expand price cuts under the Cost-Effectiveness Assessment (CEA) system, and Chuikyo supported this direction. JPMA, PhRMA and EFPIA are issuing this joint statement to share our concerns and to urge the Japanese government to reconsider.

In July, we welcomed the Takaichi Administration’s Basic Policy for Economic and Fiscal Management and Reform (Honebuto) and Japan Growth Strategy. These policies position the innovative biopharmaceutical industry as a strategic sector. They set a goal of restoring Japan’s patented medicines market to 9.6% annual growth, comparable with leading markets, and commit to further recognition of innovation in prices at initial National Health Insurance listing and during the patent period.

To help realize these goals, we proposed concrete measures at the Chuikyo hearing on August 26. Japan should reform its initial price-setting rules to align with U.S. price levels after adjusting for purchasing power parity. Japan should also freeze the rules that prevent the maintenance of prices during the patent period, including market price revisions, market expansion re-pricing and the CEA system. In practice, the CEA system has operated as a means to cut prices. Of the 57 patented medicines that have completed mandatory assessments, 45 (79%) had their prices cut, while none had their prices increased.

In parallel, we participated in good faith in an “objective verification” process regarding the CEA system from May to August and identified unresolved systematic and technical issues. Nevertheless, the report summary presented to Chuikyo on September 9 concluded that the assessments have “a certain degree of validity.” We respectfully disagree and question the objectivity of the verification that led to this conclusion.

At the Chuikyo hearing on September 16, we highlighted that the basis and scope of this conclusion had not been made clear. Given that institutional discussions remain incomplete, we requested to freeze price cuts under the CEA system for the time being. However, MHLW treated the conclusion as settled and proposed to proceed with discussions premised on expanding the price cut range. A decision of this significance deserves more careful and transparent deliberation.

We are therefore deeply disappointed by the MHLW proposal and Chuikyo support for it. Expanding the CEA system would further reduce prices during the patent period, which is difficult to reconcile with the government’s goal of growing the patented medicines market. We are also concerned that the proposal is moving forward without any substantive discussion evaluating its consistency with the positive direction set by the Honebuto and the Japan Growth Strategy.

To achieve the goals of the Takaichi Administration, Japan needs to strengthen incentives for innovation and contribute its fair share for innovative medicines. This has become even more important as U.S. Most-Favored-Nation policies reshape global incentives for developing and launching new medicines in Japan. Further price reductions during the patent period risk worsening drug loss and weakening Japan's attractiveness for investment, running directly counter to the development of the biopharmaceutical industry as a strategic sector for economic growth. We urge MHLW not to implement this expansion and to pursue thorough discussions consistent with the government’s policy direction.
