日本の特許医薬品市場成長目標の達成に向けた迅速な対応を求める

本日、高市政権は「経済財政運営と改革の基本方針(骨太の方針)」および「日本成長戦略」を決定しました。PhRMAは、革新的医薬品産業が、サポーティブな政策支援を行う戦略分野であり、かつ今後の魅力ある成長市場として位置付けられたことを歓迎します。具体的には以下が含まれます。
• 特許期間中の医薬品について、新規収載時および特許期間中におけるイノベーションの更なる評価を検討すること
• 日本の特許医薬品市場の成長率を主要先進国市場に匹敵する年率9.6%の水準に回復させること
• 米国の最恵国待遇(MFN)価格政策の提案を踏まえてもなお、日本国内で新薬の上市が継続的に促進される環境を確保すること
これらの目標を実現し、日本が革新的医薬品に係る世界全体のコストを公平に分担するため、PhRMAは、本年中に以下の薬価制度改革に着手することを求めます。
• 第一に、特許期間中の医薬品の初期薬価を GDP 調整後の米国価格と整合する水準に設定すること
• 第二に、特許期間中の医薬品の市場実勢価改定、市場拡大再算定(特例拡大再算定など)、費用対効果評価制度を含む、特許期間中の医薬品の薬価維持を妨げる制度を廃止すること
あわせて、骨太の方針は、2027年度薬価改定の実施と、費用対効果評価制度の見直しについて一定の結論を出すことも明記しています。PhRMAは、特許期間中の医薬品が2027年度改定の対象から除外され、費用対効果評価制度の拡大が取り止められるよう、これらの議論に積極的かつ建設的に参画してまいります。いずれの施策も、特許期間中の医薬品の薬価を更に引き下げるものであり、日本の特許医薬品市場の魅力を高めるという政府自らが掲げた目標を損なうものです。
「骨太の方針」および「日本成長戦略」の決定は、7月10日に開催された「創薬力向上のための官民協議会」における高市総理の前向きなご発言を受けたものと認識しています。PhRMAは、総理が、特許期間中の医薬品に関する日本のイノベーション・エコシステムおよび薬価政策の改善、革新的医薬品が日本の患者さんに迅速に届くようMFN価格政策へ対応すること、そして官民協議会の下に具体的な協議のための新たな場を設置することを表明されたことを評価します。
政策の方向性が示された今、PhRMAは、総理の表明を本年早いうちに具体的な改革へと結実させるための議論に積極的に参画してまいります。大胆な改革を通じてこそ、日本は、経済成長を牽引する国内外からの投資を呼び込み、ドラッグ・ラグおよびドラッグ・ロスを解消し、ひいては日本の患者さんの命を守り、健康増進に資することができると信じております。

PhRMA Welcomes Honebuto and Japan Growth Strategy,
Calls for Swift Action to Achieve Patented Medicine Market Growth Target
The Pharmaceutical Research and Manufacturers of America (PhRMA) represents the world’s leading biopharmaceutical research companies operating in Japan. We support an innovation ecosystem that encourages the discovery of medicines that transform lives, and where we work as trusted partners with the Japanese government so that patients can access the treatments they need.
Earlier today, the Takaichi Administration released the Basic Policy for Economic and Fiscal Management and Reform (Honebuto) and the Japan Growth Strategy. PhRMA welcomes the innovative biopharmaceutical industry’s inclusion as a strategic sector in need of supportive policies and an attractive market, including:
• Consider further recognition of innovation for patented medicines at initial NHI price listing and during the patent period.
• Restore Japan’s patented medicines market to 9.6% annual growth, a rate comparable with leading markets.
• Ensure an environment that encourages continued launches of new medicines in Japan given U.S. Most-Favored-Nation (MFN) pricing policy proposals.
To realize these goals and ensure that Japan contributes fairly to the global costs of biopharmaceutical innovation, we recommend action this year on the following pricing rule reforms:
• First, Japan should improve how it sets initial prices for patented medicines by aligning with U.S. price levels after adjusting for GDP.
• Second, Japan should abolish rules that prevent the maintenance of prices during the patent period, including market price revisions, market expansion repricing and related policies (e.g., huge-seller repricing), and cost-effectiveness assessments.
The Honebuto also states that Japan will implement an FY2027 drug pricing revision and conclude the ongoing cost-effectiveness assessment system review. PhRMA will engage actively and constructively in these discussions so that patented medicines are excluded from the FY2027 revision and the cost-effectiveness assessment system is not expanded. Both policies would further cut prices during the patent period and undermine the government’s own goal to enhance the attractiveness of Japan’s patented medicines market.
The release of the Honebuto and the Japan Growth Strategy follows the supportive remarks delivered by Prime Minister Takaichi at the Public-Private Council (PPC) for Enhancing Drug Discovery Capability on July 10. We recognize and appreciate that the Prime Minister committed to improve Japan’s innovation ecosystem and pricing policies for patented medicines, respond to U.S. MFN pricing policies so that innovative medicines reach Japanese patients without delay and establish a new forum for concrete discussions under the PPC.
With the policy direction now set, PhRMA will participate actively in discussions to translate the Prime Minister’s commitments into reforms this year. Only through bold reforms will Japan attract domestic and global investment that drives economic growth, resolves drug lag and loss, and ultimately advances the health of Japanese patients.
